Nutrition for Surgical Menopause: What to Eat After an Oophorectomy

If you’ve recently had an oophorectomy or you’re preparing for one, you’re probably navigating a lot of information, some of it conflicting, much of it overwhelming. As a registered dietitian with 19 years of clinical experience, I’ve worked with many women in exactly this position. This post is what I’d want every one of them to read first.


Surgical menopause where both ovaries are removed before natural menopause triggers an abrupt hormonal shift that brings specific, well-documented nutritional challenges. As a registered dietitian with nearly two decades of experience supporting women through complex health, here’s what the evidence actually says about nutrition after an oophorectomy.

What is surgical menopause?

Surgical menopause happens when both ovaries are removed, a procedure called a bilateral oophorectomy, before natural menopause occurs. It can happen as part of a hysterectomy, as a risk-reducing surgery for women carrying BRCA1 or BRCA2 gene mutations, or as part of treatment for conditions like endometriosis, ovarian cancer, or cervical cancer.
Unlike natural menopause, which unfolds gradually over several years, surgical menopause is immediate. Oestrogen levels fall sharply within days of surgery, and this abruptness is what makes the experience so distinct and often so intense.
Understanding how nutrition can support your body through this is one of the most empowering steps you can take. It’s something I’m asked about constantly by clients, by women in my online community, and by healthcare professionals looking for reliable, evidence-based resources to share.

Why Surgical Menopause Is Different: Key Health Risks

One of the things that frustrates me most in the online menopause space is how rarely this distinction is made. Surgical menopause is not simply “early menopause.” The abruptness of the hormonal shift has specific, well-documented health implications that are more pronounced than those seen with a natural transition.

After 19 years of working across clinical and nutrition settings, I can tell you that women who’ve had an oophorectomy often feel that standard menopause advice doesn’t quite fit their experience because it doesn’t. Research confirms that surgical menopause is associated with:

  • Accelerated bone loss โ€” Studies indicate that women who undergo oophorectomy lose bone at a faster rate than those who reach menopause naturally, at least in the first few years post-surgery. One study found cumulative spinal bone loss of around 25.8% in women with surgical menopause, compared to 21.8% in natural menopause, over the same period.
  • Increased cardiovascular risk โ€” The loss of oestrogen’s cardioprotective effects happens abruptly, increasing risk of heart disease and stroke.
  • Greater vasomotor symptom burden โ€” Hot flushes, night sweats, and sleep disruption can be more severe when oestrogen drops suddenly rather than gradually.
  • Cognitive and mood changes โ€” There is emerging evidence of increased risk of anxiety, depression, and cognitive changes following early surgical menopause.

This is why nutrition matters so much here and why generic healthy eating advice, while not wrong, isn’t the full picture.

Hormone replacement therapy (HRT) is first-line treatment for most women with surgical menopause, and diet works alongside it, not instead of it. If you haven’t yet had a conversation with your GP or menopause specialist about HRT, that conversation is worth having.

Nutrition After Oophorectomy: 7 Clinical Priorities

In my clinical work, I approach surgical menopause nutrition through several interconnected priorities. Here’s what I focus on with clients, and why.

1. Bone health: calcium and vitamin D

With bone loss accelerated after oophorectomy, getting adequate calcium and vitamin D is non-negotiable. This is one of the areas where I see the biggest gap between what women are told and what they actually need and one where the supplement market creates real confusion.

Calcium: The UK recommended intake for adult women is 700mg per day, but in my clinical practice I’d be looking to see women post-oophorectomy hitting closer to 1,000โ€“1,200mg, in line with what many menopause specialists advocate. The goal is to get the majority from food rather than supplements, as food-based calcium is better absorbed and has a more reassuring safety profile.

Good food sources of calcium include:

  • Dairy (milk, yoghurt, cheese)
  • Canned sardines and salmon eaten with bones
  • Fortified plant milks (check the label โ€” not all are fortified equally)
  • Tofu set with calcium sulphate
  • Sesame seeds and tahini
  • Kale, pak choi, and broccoli (note: spinach is high in oxalate which limits calcium absorption)

Vitamin D: Without adequate vitamin D, calcium absorption is severely limited. In the UK, sunlight exposure between October and March is insufficient for vitamin D synthesis, and Public Health England recommends all adults consider a supplement of 10 micrograms (400 IU) daily throughout the year. If you have confirmed deficiency, your GP may prescribe a higher therapeutic dose.

Food sources of vitamin D are limited but include oily fish (salmon, mackerel, herring), egg yolks, and fortified foods. Supplementation is almost always necessary in the UK.

A note on calcium supplements: Some evidence has raised questions about high-dose calcium supplementation and cardiovascular risk. In my practice, this is exactly why I work with clients to audit dietary calcium first, using supplements only to bridge a genuine, identified shortfall, never as a default.

2. Protein: protecting muscle mass and bone

This is an area I feel strongly about, and one that’s consistently underemphasised in general menopause advice. Oestrogen plays a role in muscle protein synthesis, so its sudden loss accelerates the natural age-related decline in muscle mass (sarcopenia). Muscle and bone health are deeply interconnected, building one supports the other.

In my clinical work, I frequently see women eating far too little protein, particularly at breakfast, because they’ve been so conditioned to think of protein as belonging to “men’s fitness.” That thinking needs to change.

Current evidence suggests women going through menopause benefit from protein intakes above the basic RNI of 0.75g per kg body weight. I’d typically aim for 1.0 – 1.2 per kg per day with clients post-oophorectomy, distributed across meals, with each main meal containing a meaningful source of around 25โ€“30g.

Spreading protein across the day matters. Research supports that protein synthesis is optimised when intake is distributed evenly, with each meal providing enough to meaningfully stimulate muscle building.

Good protein sources:

  • Meat, poultry, and fish
  • Eggs
  • Dairy (Greek yoghurt, cottage cheese, milk)
  • Legumes: lentils, chickpeas, beans
  • Tofu and tempeh
  • Edamame

Combining adequate protein with resistance exercise (strength training) is far more effective than either alone. Even two sessions per week makes a meaningful difference, something I encourage all my menopause clients to explore alongside their dietary changes.

3. Heart health: fat quality, fibre, and overall dietary pattern

The cardiovascular risks associated with surgical menopause make heart-protective eating a genuine clinical priority. In 19 years of practice across both NHS and private settings, I’ve seen how much diet can move the needle on cardiovascular risk markers and also how much confusion exists about which dietary changes actually matter.

Fat quality matters. Replacing saturated fat (found in fatty cuts of meat, butter, full-fat dairy, coconut oil, and ultra-processed foods) with unsaturated fats supports healthy cholesterol levels and reduces cardiovascular risk. Good sources of unsaturated fats include:

  • Oily fish (also rich in anti-inflammatory omega-3 fatty acids)
  • Extra virgin olive oil
  • Avocado
  • Nuts and seeds

A 2023 meta-analysis found that omega-3 fatty acid supplementation in postmenopausal women was associated with a significant reduction in triglyceride concentrations and a modest elevation of HDL (“good”) cholesterol. Including oily fish two to three times per week is a solid starting point; supplementation can be considered if intake is consistently low.

Dietary fibre plays multiple roles in cardiovascular protection: it supports healthy cholesterol levels, improves blood sugar regulation, feeds the gut microbiome, and reduces systemic inflammation. Aim for at least 30g of fibre per day from a variety of sources including vegetables, fruit, pulses, whole grains, nuts, and seeds.

The overall dietary pattern counts more than any single food. This is something I come back to repeatedly with clients who are focused on chasing individual superfoods: no one ingredient compensates for a poor overall diet. The Mediterranean dietary pattern which is rich in vegetables, fruits, whole grains, legumes, oily fish, olive oil, and nuts, has the strongest evidence base for heart health in menopausal women. It’s the framework I reference most often, in my clinical practice.

4. Phytoestrogens: the soy question

I get asked about soy constantly, mostly by women who’ve been told to avoid it entirely, often without any clinical basis for that advice. Let me address this directly, because the misinformation in this space is genuinely unhelpful.

Phytoestrogens are plant compounds that weakly interact with oestrogen receptors in the body. The most studied are isoflavones, found primarily in soy foods, and lignans, found in flaxseeds, wholegrains, and legumes.

The evidence on phytoestrogens and hot flushes is mixed but moderately encouraging with some systematic reviews suggesting modest reductions in flush frequency and severity, particularly with higher isoflavone intakes. They are not a replacement for HRT, but they may offer some additional support, particularly for women who cannot use HRT.

Are soy foods safe? For the majority of women, including most breast cancer survivors. Current evidence does not support the concern that moderate soy food consumption increases breast cancer risk. In fact, some evidence suggests it may be protective. The caveat is for women with oestrogen receptor-positive (ER+) breast cancer, for whom I’d always recommend seeking specific guidance from their oncology team before making changes.

Practical sources of soy isoflavones include:

  • Edamame
  • Tofu
  • Tempeh
  • Soy milk
  • Miso

Whole soy foods are preferable to concentrated isoflavone supplements, where evidence and safety data are far less robust.

Flaxseeds (linseeds) are a good source of lignans and also provide omega-3 fatty acids and fibre. A tablespoon of ground flaxseed in porridge or yoghurt is one of the simplest, most evidence-informed additions I recommend.

5. Blood sugar and weight management

Many women notice changes in body composition and weight after surgical menopause, even without changing their diet. This is real, it’s well-documented, and it is not a personal failure. Reduced oestrogen affects fat distribution (shifting it toward the abdomen), slows metabolic rate, and can disrupt insulin sensitivity.

What I see frequently in practice is women responding to these changes with severe restriction which creates a different set of problems entirely. Under eating accelerates muscle and bone loss, disrupts sleep, worsens mood, and ultimately makes weight management harder over time.

Instead, the focus should be on:

  • Stabilising blood sugar through regular meals containing protein, fibre, and healthy fats. Avoiding prolonged gaps between meals helps prevent the energy crashes and cravings that often lead to poorer food choices later in the day.
  • Reducing ultra-processed foods and refined sugars, which drive inflammation and disrupt metabolic health.
  • Including whole grains over refined carbohydrates โ€” oats, quinoa, brown rice, wholegrain bread, barley.
  • Eating enough. Adequate energy intake supports bone turnover, muscle maintenance, sleep quality, and mood. Getting this balance right is exactly what I work through with clients on an individual basis.

6. Gut health

There’s an emerging and genuinely fascinating relationship between the gut microbiome and oestrogen metabolism. A community of gut bacteria known as the “estrobolome” produce an enzyme that influences oestrogen recycling in the body. After surgical menopause, with oestrogen levels dramatically reduced, supporting gut microbiome diversity takes on added significance.

This is an area of active research that I follow closely in my clinical and consultancy work. The practical takeaway, though, is straightforward: a fibre-rich diet that includes a wide variety of plant foods is the strongest driver of microbiome diversity. Fermented foods such as yoghurt, kefir, kimchi, sauerkraut, may also offer benefits.

There is no supplement-based shortcut here, whatever the wellness industry would have you believe.

7. Managing hot flushes through diet

While diet alone won’t eliminate vasomotor symptoms, and I want to be honest about that, certain choices can meaningfully reduce their frequency and intensity. This is something I discuss with every menopause client I work with.

Potential triggers to be aware of:

  • Alcohol (particularly red wine โ€” a well-established and potent hot flush trigger)
  • Caffeine in some women
  • Very spicy foods
  • Large meals

Dietary choices that may help:

  • Soy-based foods (see phytoestrogens section above)
  • Flaxseeds
  • Maintaining a healthy weight (excess adipose tissue is consistently associated with more frequent and severe hot flushes)
  • Keeping blood sugar stable through regular, balanced meals

Supplements for Surgical Menopause: What’s Worth It?

The surgical menopause supplement market is crowded, expensive, and largely poorly evidenced. Having worked in both clinical practice and the supplement industry as a consultant, I have a fairly clear-eyed view of this space.

In the context of surgical menopause, the supplements worth a targeted conversation with your GP or dietitian are:

  • Vitamin D โ€” widely recommended across the UK population; even more important post-oophorectomy
  • Calcium โ€” only if dietary intake is consistently and genuinely low, following a proper dietary assessment
  • Omega-3 (EPA/DHA) โ€” if oily fish intake is consistently low, particularly given the cardiovascular risk context
  • Magnesium โ€” involved in bone health and may support sleep quality; frequently low in typical Western diets

My starting point with clients is always to assess actual dietary intake first. Supplements fill specific, identified gaps, they don’t replace a nutritious diet, and investing significant money in a supplement bundle before understanding your dietary baseline is rarely the right move.

HRT and Nutrition After Surgical Menopause

For most women with surgical menopause, HRT is strongly recommended, not just for symptom management, but for long-term protection of bone, cardiovascular, and cognitive health. The British Menopause Society is clear that the benefits of HRT in women with premature surgical menopause outweigh the risks for the majority of women.

Nutrition and HRT are complementary, not competing. HRT addresses the underlying hormone deficit; a well-constructed diet provides the nutritional foundation that supports bone remodelling, cardiovascular function, gut health, and energy. They are most powerful together and that’s the approach I take with clients.

If you’ve been told you must manage without HRT, it is worth seeking a second opinion from a menopause specialist particularly if you had surgery before the age of 45.

Frequently asked questions about nutrition and surgical menopause

How much protein do I need after an oophorectomy? Based on current evidence and my clinical practice, I’d typically aim for 1.0โ€“1.2g of protein per kg of body weight per day, spread across meals. For a 65kg woman, that’s roughly 60โ€“80g of protein per day, significantly more than the basic UK recommendation.

Can I eat soy after breast cancer? For most women, yes. If your cancer was oestrogen receptor-positive, I’d recommend discussing soy intake specifically with your oncologist before making changes.

Will a plant-based diet give me enough calcium and protein? A well-planned plant-based diet can meet both needs, but it requires genuine thought and nutritional knowledge. Calcium-set tofu, fortified plant milks, edamame, and legumes all contribute. Protein adequacy needs specific attention, and B12 and vitamin D supplementation is essential. This is an area where working with a registered dietitian is genuinely worth the investment.

Should I take a menopause supplement? Most are not well-evidenced. I’d always recommend assessing dietary gaps first. Vitamin D is worth supplementing across the UK population year-round. Before investing in a bundle of supplements, a conversation with a registered dietitian will save you time, money, and confusion.
